  • Educational or medical leave of absence (Be careful – close the loop! – require a report)

  • Mutual release
  • Language for responding to credentialing inquiries
  • Data Bank and state reports
  • Waiver of future hearing rights
  • Money – always a sore point! (Usually it is for legal fees)

  • Quality issues are hard to mediate
