Tracy Meure RN, RSN

Tracy Meure is the Clinical Director for Meadville Medical Center’s Community Care Network (CCN). She is a registered nurse and received her BSN at Edinboro University of Pennsylvania.
Tracy’s work experience was concentrated in the care of end stage renal disease patients for 15 years. During that time she coordinated outpatient care for patients with chronic disease and multiple co-morbidities. Her areas of expertise include clinical service, education, quality assurance, and administration. Tracy assisted in the design and opening of the first outpatient dialysis clinic geared toward ventilator dependant and morbidly obese patients. She also worked on a team at DaVita that developed and implemented the audit tool that is used nationally within their company to assess clinical survey readiness and adherence to the ESRD Conditions for Coverage.
Over the past two year she led the Community Care Network (CCN) clinical team under the direction of Barry Bittman, MD. She was instrumental in the development and implementation of the CCN utilizing unique care coordination strategies to meet the Triple Aim objectives ¾ better patient outcomes, better care experiences, and lower per capita cost.
